The office of Dr. Summakia has been closed.  Your medical records are confidential, and a copy can be transferred to another doctor or released to you or another person you designate only through your permission. Until then, your records will remain on file.

Medical Records Request to you or another person: $25.00 for the first 20 pages then $0.50 for every page after.

 

Medical Record Request to another doctor: Free of charge.
